Care Manager Registered Nurse

About the Role

This position involves providing case management services for inpatient and observation patients, assessing psychosocial, medical, and discharge needs, and coordinating care plans to ensure smooth transitions across care settings.

Responsibilities

• Identify at-risk patients
• Collaborate with healthcare teams and social services
• Evaluate care progression using evidence-based tools
• Maintain effective communication with physicians, patients, families, and external agencies

Requirements

• Minimum of 2+ years in Case Management or 3+ years in a clinical setting
• RN license
• Associates Degree in Nursing (Master’s preferred)
• BLS certification
• Experience with InterQual (preferred)
