Care Manager Registered Nurse

About the role

The Case Manager supports patients and families throughout their care journey by coordinating services, developing personalized care plans, and ensuring safe transitions across care settings. Collaborating with physicians, nurses, and department leaders, you will facilitate effective communication and quality care, assess psychosocial and medical needs, and partner with social services to determine appropriate post‑acute care. Using evidence‑based tools like InterQual, you will evaluate care progression and escalate issues as needed. Your role involves acting as a liaison to ensure seamless communication between patients, families, hospital staff, and external agencies.

Qualifications

• Minimum 2+ years in Case Management or 3+ years in a clinical setting
• Associate’s Degree in Nursing (Bachelor’s or Master’s preferred)
• Current RN license in applicable state(s)
• Basic Life Support (BLS) certification
• Experience with InterQual is preferred
