About the role

Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Responsibilities

  • Facilitate communication and care coordination among patients, caregivers, and healthcare providers.
  • Assess and address the social determinants of health impacting patient care.
  • Collaborate with interdisciplinary teams to develop and implement care plans.
  • Coordinate discharge planning and ensure seamless transitions of care.
  • Provide support and resources to patients and families.
  • Engage in ongoing professional development and stay updated on best practices in care management.

Requirements

  • Master's Degree in Social Work.
  • Coursework/Training in All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
  • California Clinical Social Worker License.
  • Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluations.
  • Within 1 year of hire: CPI certification (nonviolent crisis intervention training).
  • 2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
  • Computer experience.

Qualifications

  • Discharge planning experience.

Skills

  • Excellent communication and interpersonal skills.
  • Strong organizational and problem-solving abilities.
  • Ability to manage multiple tasks and prioritize effectively.
  • Knowledge of social determinants of health and their impact on patient care.
  • Proficiency in Microsoft Office Suite.

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Schedule

This position is Full time and will work 8-hour, Day shifts.
