Care Manager - Home Delivered Meals
About the role
Westchester Jewish Community Services (WJCS) is an award-winning, nonprofit, nonsectarian human services organization founded in 1943. Today, it is the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. WJCS delivers innovative programs and services and compassionate care to more than 20,000 people of all ages and backgrounds throughout the county.
We are currently seeking a compassionate and organized full-time (35 hours per week) Care Manager to join our Home Delivered Meals Program. In this role, you will offer services, including in-home assessments, case management, care planning, information and referrals to the seniors participating in the program.
Responsibilities
- Conduct comprehensive in-home assessment, including financial assessments, to determine eligibility for the grant, programs, services and entitlements.
- Complete home visits throughout the Westchester County community.
- Conduct annual reassessments, as well as reassessments whenever necessary.
- Develop individualized client Care Plans.
- Ensure clients and their caregivers receive information about and assistance with appropriate referrals.
- Maintain accurate, timely and complete documentation in accordance with agency program requirements.
Requirements
- Bachelor’s Degree in a human services field
- Experience working with senior citizens
- Bilingual English/Spanish is a plus
- Valid driver’s license and vehicle
- Computer literacy
