Care Manager - High Fidelity Wraparound

Responsibilities

• Provide care coordination services to youth/adolescents, up to age twenty-one.
• Follow, to fidelity, the HFW process and procedures set forth by OMH.
• Provide crisis intervention and prevention services as necessary.
• Cook up, the care for program participants and work closely with a variety of providers and family members to ensure the youth's success and positive mental wellness.
• Maintain records to according to agency policies and procedures and to regulatory agencies directives.

Requirements

• Bachelor’s degree in Social Work or related field with a minimum of 2 years of relevant experience working with children/adolescents or a Master’s Degree with one-year experience.
• Ability to work with youth/adolescents/young adults from 5 to 21 years old who are impacted by serious emotional needs and/or chronic medical needs.
• Meeting facilitation skills, strong collaboration skills. Ability to engage and work with parents/caregivers.
• A driver’s license and vehicle are required.
• Bilingual English/Spanish required.
