Care Manager

About the role

The Care Manager position at Direction Home Akron Canton is responsible for assessing and managing the care of long-term care clients enrolled on waiver programs. Applicants must be either a Licensed Social Worker (LSW) or Registered Nurse (RN) with one year of paid clinical experience in home and community-based services.

Responsibilities

• Assess and manage the care of long-term care clients enrolled on waiver programs
• Provide support and guidance to clients and their families
• Collaborate with interdisciplinary teams to ensure comprehensive care plans
• Ensure compliance with state and federal regulations

Qualifications

• Must be a Licensed Social Worker (LSW) or Registered Nurse (RN)
• One year of paid clinical experience in home and community-based services preferred
• Pre- and post-employment drug & nicotine testing required
• Proof of influenza vaccination is required
