Care Manager (CNA/CHHA) Per Diem

About the Role

This is a Per-Diem, non-exempt, hourly position reporting to the Health & Wellness Director. All shifts are available.

Salary Range: $22.00 hourly.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding, as outlined in the resident’s care plan.
• Maintain the safety, comfort, dignity, and confidentiality of all residents.
• Escort residents to meals and activities.
• Ensure all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during your shift.
• Report any change of condition in a resident immediately to your supervisor.

Requirements

• Certified Nursing Assistant or Certified Home Health Aide.
• Knowledge and experience in working with the elderly population; dementia experience preferred.

Physical Abilities

• Frequently stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• Lift or carry weight up to 50 lbs.
• Perform intermittent physical activity including lifting and supporting residents.
• Use proper body mechanics.
