Care Manager

About the role

Directly engage with clients and their family members to support home care services and community-based care management.

Responsibilities

• Conduct professional assessments to identify physical, emotional, social, and spiritual needs.
• Develop and manage comprehensive care plans to ensure stability, clarity, and peace of mind for clients.
• Participate in local committees, task forces, and boards to represent the organization in the community.
• Engage with professionals such as attorneys, physicians, and trust officers to educate and assist them in providing better care to their clients.
• Communicate confidently about the value of care management services, conveying a sense of support and assistance.

Requirements

• Minimum educational requirement: Bachelor’s Degree, preferably in a healthcare-related field.
• Prior experience in care management and working with the aging population is preferred.
• Excellent written and verbal communication skills.
• Valid driver’s license and proof of current auto insurance.

Skills

• Ability to interface effectively with clients and their families, as well as with professional service providers.
• Teamwork and collaboration skills.
• Knowledge of healthcare-related fields such as social work, nursing, gerontology, psychology, health and human services, occupational therapy, and related areas.
