About the role
Directly engage with clients and their family members to support home care services and community-based care management.
Responsibilities
- Conduct professional assessments to identify physical, emotional, social, and spiritual needs.
- Develop and manage comprehensive care plans to ensure stability, clarity, and peace of mind for clients.
- Participate in local committees, task forces, and boards to represent the organization in the community.
- Engage with professionals such as attorneys, physicians, and trust officers to educate and assist them in providing better care to their clients.
- Communicate confidently about the value of care management services, conveying a sense of support and assistance.
Requirements
- Minimum educational requirement: Bachelor’s Degree, preferably in a healthcare-related field.
- Prior experience in care management and working with the aging population is preferred.
- Excellent written and verbal communication skills.
- Valid driver’s license and proof of current auto insurance.
Skills
- Ability to interface effectively with clients and their families, as well as with professional service providers.
- Teamwork and collaboration skills.
- Knowledge of healthcare-related fields such as social work, nursing, gerontology, psychology, health and human services, occupational therapy, and related areas.
