Key Responsibilities
- Perform comprehensive assessments of clients’ health, living conditions, and overall needs.
- Collaborate with families and healthcare professionals to create and implement individualized care plans.
- Advocate for clients within the healthcare system to ensure they receive high-quality, timely care.
- Lead efforts in marketing and intake to attract new clients and referrals, highlighting the quality of services offered by Reflections Management and Care.
- Provide ongoing support and guidance to clients and families, ensuring care plans are adjusted as needed.
Position Goals
- Enhance the quality of life for clients through comprehensive and compassionate care management.
- Use your expertise to address the unique needs of each client and advocate for optimal care solutions.
- Continuously identify and implement strategies for improving care delivery and client satisfaction.
- Analyze performance data to refine and optimize care plans for maximum impact.
Qualifications
- Master’s in Social Work, RN, or a similar degree with relevant care management experience.
- Minimum of 5 years of experience in senior care, with a preference for those with a background in care management, social work, or hospice care.
- Certification by the National Academy of Certified Care Managers is a plus.
- Experience working with individuals with Alzheimer’s disease or other forms of dementia.
- Strong written and verbal communication skills with the ability to work independently and as part of a team.
- Reliable transportation for client visits (travel is a requirement as we serve many counties in the surrounding area).
- A commitment to maintaining high standards of care and professionalism.
Requirements
- Profound understanding of aging processes, psychosocial issues, medical procedures, and medication management.
- Strong leadership, supervisory, and interpersonal communication skills.
- In-depth knowledge of healthcare systems, medical terminology, and the ability to navigate complex care scenarios.
