About the role
Work directly with clients and their family members; provide support for home care services and community-based care management services. Conduct professional assessments to understand physical, emotional, social, and spiritual needs, develop a care plan, and manage it.
Responsibilities
- Conduct professional assessments to understand physical, emotional, social, and spiritual needs, develop a care plan, and manage it.
- Be our face in the community: Participate on local committees, task forces, and boards. Tasks may include education / presenting to professional groups; raising consciousness and helping professionals learn how we can help them help their clients/patients; setting up meetings with professionals, getting to know them, figuring out what they need/want, and helping create solutions.
Requirements
- Education: a Bachelor’s Degree. Preferred: Master’s Degree in a healthcare-related field.
- Experience: A minimum of two (2) years experience in care management, experience working with the aging population preferred; other professional experience may substitute.
- Skills: Excellent written and verbal communication skills required. PC literacy required. Valid driver’s license and proof of current auto insurance required.

Skills
- Ability to interface effectively and professionally with client support, including family, physicians, attorneys, etc.
