Care Manager - CCBHC

Responsibilities

• Provides community-based services to consumers and consumer’s family in the community
• Facilitates service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers
• Aids and supports individuals in developing skills to gain access to needed medical, behavioral health, housing, employment, social, educational and other services essential to meeting basic needs
• Supports individuals in the use of basic community resources such as transportation and financial resources
• Ensures that individuals needs and goals are addressed, including aggressive advocacy, escort to appointment, daily living skill remediation and money management
• Accurately and timely documents services in the electronic health record
• Provides 24 hours/7 days per week on call crises intervention, including in person response on a rotating basis, including evenings and weekends
• Collaborates with the Clinical Team

Qualifications

• Bachelor’s degree in a related field
• One year experience working in a behavioral health setting preferred
