Care Manager - Cannonsburg and Bridgeville, PA

Crossroads is a family of professionals dedicated to providing the most accessible, high-quality, evidence-based medication assisted treatment (MAT) options to combat the growing opioid epidemic and help people with opioid use disorder start their path to recovery. Our comprehensive approach—the gold standard in care—has been shown to prevent overdose deaths and lead to long-term recovery. We are committed to bringing critical services to communities across the U.S. to improve access to treatment for over 26,500 patients. Our clinics are all outpatient and office-based, located in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia.

About the role

A day in the life of a Care Manager includes:

• Connecting with patients monthly to address any needs and provide resources to fulfill those needs.
• Building and maintaining relationships with community partners for patient resources.
• Working closely with a dedicated team to provide exceptional patient care.
• Participating in community awareness and engagement events to promote Crossroads, our mission, principles, and approach to addiction treatment.

Requirements

• At minimum, a High School Diploma or GED.
• Experience in a fast-paced environment servicing many clients is preferred.
• Experience in healthcare, addiction medicine, community health, or care coordination preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices.
• Lived addiction experience is a plus.

Schedule

Care Managers work within a localized geographical area where we have multiple locations, including Canonsburg and Bridgeville. Expected hours for this role are 37-40 hours per week with few opportunities for overtime. Clinic hours range from 6:30 AM – 9:00 PM. Travel is required to Canonsburg and Bridgeville clinics.

Tentative schedule:

• Tuesday: Canonsburg 8:00 AM - 3:30 PM
• Wednesday: Bridgeville 8:00 AM - 4:30 PM
• Thursday: Admin Day
• Friday: Bridgeville 8:00 AM - 3:30 PM
• Saturday: Canonsburg 7:00 AM - 1:30 PM
• Every Other Sunday: Bridgeville 7:00 AM - 1:30 PM
