Care Manager - Erie, PA

Our clinics are all outpatient and office-based, with locations in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia.
About the role
A day in the life of a Care Manager includes:
- Connecting with patients monthly to address any needs and provide the resources to fulfill those needs.
- Connecting and maintaining relationships with community partners for patient resources.
- Working closely with a dedicated team to provide exceptional patient care.
- Participating in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
Requirements
- At minimum, High School Diploma or GED required.
- Experience in a fast-paced environment, servicing many clients, is preferred.
- Experience in healthcare, addiction medicine, community health, or care coordination preferred.
- Experience with building individualized care plans is a plus.
- Knowledge of compliant documentation practices.
- Lived addiction experience is a plus.
Schedule
Expected hours for this role are 37-40 hours per week with few opportunities for overtime. Clinic hours range anywhere from 6:30 AM – 9:00 PM. Team members are required to rotate weekend shifts. Business hours vary by location. Example hours for the Erie clinic:
- Saturday: 8:00 AM - 4:30 PM
- Sunday: 7:00 AM - 1:30 PM
- Monday: 7:00 AM - 2:30 PM
- Tuesday: 7:30 AM - 3:00 PM
- Wednesday: 7:30 AM - 3:00 PM
- Thursday: 8:00 AM - 5:30 PM
- Friday: 7:00 AM - 1:00 PM
