Care Manager - (Bilingual Spanish or Mandarin)

Responsibilities

• Participates in outreach and enrollment activities
• Assists and guides individuals, family members, and caregivers on available services
• Serves on an inter-disciplinary team to provide service coordination
• Manages the medical, social, and behavioral health needs of enrolled members
• Oversight and facilitation of access to all necessary services for health, prevention of hospitalizations, and stability
• Conducts recruitment efforts and actively monitors waiting lists for accurate placement
• Completes intake, enrollment, and ongoing assessments
• Facilitates interdisciplinary collaboration among providers, enrollees, families, caregivers, and available supports
• Awareness of all support services and entitlements for families
• Provides recommendations and referrals to relevant community resources
• Advocates for individuals/families by serving as a liaison between individual/parents and other programs
• Conducts home visits for assessing family needs and service delivery
• Assists in coordination of services for special needs, providing support to families with disabled children
• Maintains compliance with all Federal, State, and Local regulations
• Maintains compliance using data management systems

Requirements

• Bachelor’s degree in Human Services or a related field
• 2 years of experience working in Human Services, Psychology, or related fields that included course work in the principles of social work, child development, counseling, or psychology
• Ability to plan and carry out assignments independently
• Ability to prioritize, adhere to timelines, and multi-task
