Responsibilities
- Participates in outreach and enrollment activities
- Assists and guides individuals, family members, and caregivers on available services
- Serves on an inter-disciplinary team to provide service coordination
- Manages the medical, social, and behavioral health needs of enrolled members
- Oversight and facilitation of access to all necessary services for health, prevention of hospitalizations, and stability
- Conducts recruitment efforts and actively monitors waiting lists for accurate placement
- Completes intake, enrollment, and ongoing assessments
- Facilitates interdisciplinary collaboration among providers, enrollees, families, caregivers, and available supports
- Awareness of all support services and entitlements for families
- Provides recommendations and referrals to relevant community resources
- Advocates for individuals/families by serving as a liaison between individual/parents and other programs
- Conducts home visits for assessing family needs and service delivery
- Assists in coordination of services for special needs, providing support to families with disabled children
- Maintains compliance with all Federal, State, and Local regulations
- Maintains compliance using data management systems
Requirements
- Bachelor’s degree in Human Services or a related field
- 2 years of experience working in Human Services, Psychology, or related fields that included course work in the principles of social work, child development, counseling, or psychology
- Ability to plan and carry out assignments independently
- Ability to prioritize, adhere to timelines, and multi-task
