About the role

- Set up meetings with professionals and assist in creating solutions.
Requirements
- Minimum Bachelor’s Degree, Master’s Degree preferred in healthcare-related fields.
- Experience in care management, preferably with the aging population.
- Excellent written and verbal communication skills.
- Valid driver’s license and proof of current auto insurance.
Skills
- Confidence in the value of Care Management services.
- Ability to work effectively with clients, family members, and professional service providers.
- Teamwork and collaboration skills.
