About the role

Directly engage with clients and their family members to support home care services and community-based care management.

Conduct professional assessments to understand physical, emotional, social, and spiritual needs, and develop a care plan.

Engage with the community through participation on local committees, task forces, and boards.

Responsibilities

• Set up meetings with professionals and assist in creating solutions.

Requirements

• Minimum Bachelor’s Degree, Master’s Degree preferred in healthcare-related fields.
• Experience in care management, preferably with the aging population.
• Excellent written and verbal communication skills.
• Valid driver’s license and proof of current auto insurance.
