About the role
Directly engage with clients and their family members to support home care services and community-based care management.
Responsibilities
- Conduct professional assessments to identify physical, emotional, social, and spiritual needs.
- Develop and manage comprehensive care plans to ensure stability, clarity, and peace of mind for clients.
- Participate in local committees, task forces, and boards to represent the organization and contribute to community initiatives.
- Engage with professionals such as attorneys, physicians, and trust officers to educate and collaborate on solutions.
Requirements
- Minimum Bachelor’s Degree, Master’s Degree preferred in healthcare-related fields.
- Experience in care management, preferably with the aging population.
- Excellent written and verbal communication skills.
- Valid driver’s license and proof of current auto insurance.
Skills
- Confidence in the value of Care Management services.
- Ability to work effectively with diverse stakeholders, including older clients, their families, and professional service providers.
