Care Manager

About the Role

Work directly with clients and their family members, providing support for home care services and community-based care management services. Conduct professional assessments to understand our client's physical, emotional, social, and spiritual needs, develop a care plan that affords a sense of stability, clarity, and peace of mind, and manage the plan.

Be our face in the community: participate on local committees, task forces, and boards. Tasks may include educating and presenting to professional groups; raising awareness and helping professionals learn how we can assist them with their clients/patients; setting up meetings with professionals, understanding their needs, and helping create solutions.

This role requires a special professional who is comfortable working with older clients and their families, as well as their satellite support providers, such as Trust Officers, Attorneys, Physicians, and other elite professional service providers. We seek someone confident in the value of Care Management services—a teammate who conveys, "I'm so glad you called us, as I know I can truly help you."

Responsibilities

• Conduct professional assessments of clients' physical, emotional, social, and spiritual needs.
• Develop and manage care plans tailored to provide stability, clarity, and peace of mind.
• Engage with the community by participating on local committees, task forces, and boards.
• Educate and present to professional groups to raise awareness of care management services.
• Build relationships with professionals (e.g., Trust Officers, Attorneys, Physicians) to understand their needs and create solutions.
• Interface effectively and professionally with client support systems, including family, physicians, and attorneys.
• Work collaboratively in a team environment.

Requirements

• Bachelor’s Degree (minimum requirement). Preferred: Master’s Degree in a healthcare-related field.
• Acceptable degrees include Social Work, Nursing, Gerontology, Psychology, Health and Human Services, Occupational Therapy, or other healthcare-related fields.
• Licensure in nursing (LVN/LPN or RN) is acceptable.
• Preferred: A minimum of two (2) years of experience in care management; experience working with the aging population is a plus. Other professional experience may substitute.
• Excellent written and verbal communication skills.
• PC literacy.
• Valid driver’s license and proof of current auto insurance.
