Responsibilities
Care Managers are responsible for managing a caseload of State Home Care consumers, conducting assessments, developing service plans, reassessing needs, and providing information and referrals.
- Ongoing assessment of older adults' physical, social, emotional, and environmental status
- Development and implementation of a service plan to address identified needs
- Reassessment and monitoring of consumer eligibility, needs, and service plan
- Arrangement for crisis intervention services
- Information and referral provision
- Collaboration with other agency departments and outside agencies
- Completion and maintenance of up-to-date documentation and data entry
- Coverage of peer caseloads during staffing shortages
- Compliance with regulatory requirements
- Participation in staff development programs
Qualifications
- Bachelor's degree in social work, human services, or a related field required
- Associates degree with significant relevant work experience may substitute for a portion of the degree
- Experience working with elders strongly preferred
- Strong written, verbal, and computer skills required
- Valid driver's license and reliable transportation required
Pay and Schedule
Competitive salaries ranging from $28.00 to $29.00 per hour, depending on experience and qualifications. Hours: 37.5 per week.
