Responsibilities

• Oversees the management of member care with members, their families or significant others and coordinates with the PCP, SCP and all of the health care team facilitating the member’s care across the continuum of care.
• Acts as a resource to staff reviewing cases, makes recommendations for change, and supervises assessments of members' needs through coordination with the care team.
• Supervises a holistic variety of member's needs through communication with the health care team.
• Ensures a high level of compliance with all documentation, provides efficacy reports, and manages the performance of direct and indirect reports.

Requirements

• Master of Social Work Degree or related field, or RN or CSW.
• Minimum one to three years previous management experience preferred.
• Previous managed care experience in the areas of utilization management and/or case management required.
• Current NYS CSW if CSW qualified.
• CCM preferred.
