About the Role

Responsible for overseeing and ensuring quality care management in accordance with guidelines and regulations set forth by the Department of Health (DOH) and Forestdale/Coordinated Children’s Services Initiative (CCF). The Health Home Care Management program ensures all clients have access to quality medical and mental health care.

Responsibilities

  • Supervise a unit of up to 5 Care Managers with a mixed acuity caseload ranging from 20-40 clients each.
  • Conduct weekly or biweekly supervision with each Care Manager to review case details, including case needs, overall well-being, and progress of each family.
  • Participate in interdisciplinary meetings, case conferences, CCF/DOH-required meetings, and other staff meetings.
  • Ensure staff attend all required trainings and remain updated with certifications (e.g., CANS-NY).
  • Facilitate team meetings to support and guide staff as needed.
  • Monitor timely submission of all documentation required by DOH in Foothold.
  • Review documentation to ensure service provision complies with DOH and CCF regulations.
  • Oversee the identification of eligible Health Home youth referred for program enrollment and consent process.
  • Coordinate and ensure execution of plans for audit preparation; participate in audits as needed.
  • Provide case coverage for the program as required.
  • Perform other duties as assigned.

Requirements

  • Master’s degree in Social Work, Nursing, Public Health, or a related field.
  • Knowledge of best practices working with children and families in health, preventive, and wellness care.
  • Bilingual (preferred).
  • Working knowledge of child welfare, developmental disabilities, mental health, and/or primary healthcare fields.
  • 2+ years of experience managing teams and/or providing care management.
  • Experience using Excel and electronic healthcare systems to review and manage data (preferred).
  • Experience providing service coordination, information, linkages, and referrals for community-based services.

  • Excellent accountability, writing, communication, and organizational skills.
  • Compassion for serving children and families.
  • Ability to travel to community-based agency offices and clients’ homes.
  • Proven decisiveness, motivational, and developmental skills; strong management abilities, including meeting deadlines, ensuring compliance with agency policies, and overseeing complete and timely maintenance of agency records in accordance with contractual requirements.

Pay

  • Master’s degree: $66,000 to $73,000
  • Master’s degree + language proficiency: $68,000 to $75,000
  • Master’s degree + license: $70,000 to $78,000
  • Master’s degree + license + language proficiency: $72,000 to $80,000
