Care Management Social Worker I

MaineHealth Behavioral Health is seeking a Clinical Social Worker to provide ongoing biopsychosocial-spiritual assessment of patients and their families or support systems. This role collaborates with interdisciplinary teams—both internal and external—to facilitate discharge planning, including complex situations, facility placement, and community resource coordination, while considering health insurance, support availability, location, and eligibility factors.

Responsibilities

• Conduct ongoing biopsychosocial-spiritual assessments of patients and families/supports.
• Collaborate with interdisciplinary teams to discuss and plan discharge options, including complex situations, facility placement, and community resources.
• Assess patient eligibility for resources based on health insurance, available support, location, and other factors.

Requirements

• Master of Social Work Degree from an accredited school.
• Licensed by the State of Maine as LMSW or LMSW-CC.
• Two years of experience in a hospital setting preferred.

Pay

The pay range for this position is $66,123.20 to $89,481.60 per year. Compensation for part-time positions is prorated based on scheduled hours and full-time equivalent (FTE).
