Job Description
- Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de‑escalation services for patients as appropriate.
- Assesses patients and families holistically for discharge planning needs in inpatient, observation, and emergency departments, including prior functioning, support systems, financial, and psychosocial factors to avoid delays.
- Reviews medical records (medications, history and physical, labs, progress notes) and incorporates clinical, social, and financial factors into transition of care plans.
- Develops discharge plans with contingency plans throughout the hospital stay, arranging post‑acute care services, facilities, and community care for social needs.
- Leverages technology and follows standard work to communicate with post‑acute care services and facilities, ensuring continuity of care, complete medical records, and accurate discharge reconciliation.
- Participates in multidisciplinary rounds to review patient status, progression, level of care, and discharge plans, identifying resources and escalating care delays to leadership as needed.
- Educates patients and families about emotional, social, and financial impacts of illness, mobilizing family/community resources and advocating for patient and family empowerment.
- Organizes and facilitates patient and family care conferences with the multidisciplinary team.
- Documents discharge planning evaluation, ongoing assessment, discharge plans, multidisciplinary rounds notes, barriers to progression, avoidable days, and patient/family needs according to standard work.
- Provides patient and family advocacy, supporting patient choice and rights during hospitalization.
- Communicates with payors for authorization of post‑acute care as needed.
- Assesses readmitted patients for perceived reasons for readmission.
- Performs other duties as assigned.

- Current working knowledge of discharge planning, utilization management, care management, performance improvement, and managed care reimbursement (Preferred)
- Knowledge of state and federal guidelines pertinent to Care Management (Preferred)
- Ability to identify appropriate community resources and collaborate with patients, families, multidisciplinary team, and community agencies to achieve desired outcomes (Preferred)
Education
Master’s degree (Required)
Licenses and Certifications
- Accredited Case Manager (ACM) – Preferred
- Certified Case Manager (CCM) – Preferred
