Care Management Manager

About the role

Responsible for the management of the department to ensure Care Management operations and workflow. Provides clear communication across the continuum to improve patient outcomes. Serves as a clinical resource and lends guidance to staff in utilization management, discharge planning, and care coordination issues. Works with Care Management Leadership and Administration in the implementation, maintenance, and evaluation of an integrated Care Management program. Evaluates and critically analyzes personnel requirements for on-the-job training, in-service education, continuing education, and orientation of newly assigned personnel. Develops staffing plans. Works closely with the Medical Director, Assistant Medical Director, and Physician Advisors.

Requirements

• Bachelor's Degree in Nursing or closely related field (Master's degree preferred)
• Three to five years of previous work-related experience
• Licensure as a Registered Nurse by the State of New Jersey or a Licensed Social Worker by the State of New Jersey
• Case Management Society of America Certification (CCM or ACM preferred)

St. Joseph’s Health provides a salary range to comply with New Jersey Law. The rate of pay for each position will be determined based on the candidate's relevant experience, qualifications, and skills. The salary range does not include incentives, differential pay, or other forms of compensation.
