About the role

The Care Management Associate (CMA) supports the Florida Long-Term Services and Supports (LTSS) Care Management team by performing non-clinical care coordination and administrative functions. The CMA assists with member outreach, care management enrollment activities, referral processing, appointment coordination, health risk questionnaires, documentation support, authorization-related activities, and data entry in care management systems.

Responsibilities

  • Conduct outbound member outreach and engagement activities.
  • Support scheduling of assessments and care management appointments.
  • Process referrals and assist with care management enrollment activities.
  • Administer questionnaires and health risk assessments as directed.
  • Provide appointment reminders and post-appointment follow-up.
  • Document activities and enter data into care management systems.
  • Respond to phone queue inquiries and route issues appropriately.
  • Support care managers with administrative and non-clinical coordination activities.
  • Aid with authorization-related tasks and other operational workflows.

Requirements

  • High School Diploma or GED required.
  • Strong communication, customer service, and organizational skills.
  • Ability to work effectively in a fast-paced care management environment.
  • Strong verbal and written communication skills.
  • Proficiency with Microsoft Office applications and data entry systems.
  • Ability to manage multiple tasks and work in a fast-paced environment.
  • Experience in customer service, healthcare, care management, Medicaid, or administrative support preferred.

Qualifications

Benefits

The typical pay range for this role is $18.50 - $35.29 per hour. This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
