Care Management Assistant
About the role
Support RN Case Managers/LMSW in meeting with patient and/or family/personal representative within 48 hours of admission (excluding weekends/holidays), to evaluate, and identify discharge needs in compliance with established guidelines and policies.
Provides support and information, as needed.
Aids in discharge planning activities to support discharge timely and efficiently through collaboration with providers and multi-disciplinary team.
Works to minimize discharge delays and achieve appropriate discharge times.
Ensures adherence to confidentiality and privacy policies.
Supports timely community resource referrals, in alignment with patients’ benefits, within EPIC as needed based on patient choice and post-hospitalization needs for discharge and transfer.
Provides appropriate linkages, referral coordination, and follow-up.
Collaborates with community liaisons and health care partners to facilitate optimal discharge plans.
Advocates for the patient's and family's needs.
Serves as a positive, professional representative of our hospital and community.

Requirements
Minimum of 2 years Acute Care Experience.
Knowledgeable of New York State and Federal Entitlement Programs and regulatory requirements.
Current Unrestricted LPN license in NY State or Social Work Degree (BSW).
Strong communication and teamwork skills.
Commitment to providing patient-first, high-quality service.
Comfort working in a fast-paced, collaborative environment.

Pay
LPN - $23.76 – $39.01 hourly, based on experience and qualifications
BSW- $21.55 – $35.38 hourly, based on experience and qualifications
