About the role
The Social Worker acts as a liaison to coordinate resources and provide psychosocial support for health plan members with complex needs, collaborating with an interdisciplinary care team to ensure comprehensive patient care and discharge planning.
Responsibilities
- Coordinate patient services
- Assess post-hospital needs
- Develop care plans
- Support patients and families
- Facilitate community resource connections
- Assist with crisis intervention and risk management
- Participate in quality improvement initiatives
- Ensure compliance with regulatory standards
Qualifications
- Bachelor’s degree in social work (BSW)
- Minimum 2 years healthcare experience
- Knowledge of healthcare terminology
- Strong communication and critical thinking skills
- Ability to work onsite in Waukesha, WI
Skills
Preferred certifications and hospital discharge experience are advantageous.
