About the role

Founded in 1993, MHC serves individuals with severe and persistent mental illness and children and adolescents with severe emotional disorders across Middle and East Tennessee. Based out of Nashville, MHC operates satellite offices in various communities.
Responsibilities
- Provide care coordination to consumers in Safety Net services.
- Continual assessment of consumer’s mental health treatment needs.
- Aid in referrals and advocacy for consumers.
- Ensure consumer’s psychiatric needs are planned and coordinated.
- Facilitate education on psychiatric diagnosis and treatment options.
- Respond to urgent/crisis consumer needs.
- Involvement of family members and friends in obtaining Safety Net services.
- Coordinate efforts with all staff and Safety Net nurse and providers.
- Aid in applying for entitlements as needed.
- Aid in Safety Net documentation.
- Aid consumers with transportation for safety net appointments.
- Keep consumer record updated with current address and coverage information.
- Aid consumers with application and renewal requirements for Safety Net coverage.
- Treat consumers and co-workers with respect.
- Develop and maintain community relations through prompt follow through with contacts.
- Meet productivity standards set forth in the Agency’s policies and procedures.
Requirements
- Bachelor’s Degree in a health-related field of counseling, psychology, social work, or other behavioral health field required.
- Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.
- Valid Tennessee Driver’s License.
- Acceptable Motor Vehicle Report (MVR).
