Care Coordinator RN (Weekdays M-F)

About the role

An RN Care Coordinator specializing in hospital care is needed to assess patient needs, coordinate care across the continuum, and facilitate discharge planning.

Responsibilities

• Conduct psychosocial and functional assessments
• Develop individualized discharge plans
• Collaborate with interdisciplinary teams
• Address barriers to timely discharge
• Manage post-acute referrals
• Ensure documentation accuracy
• Support care progression through proactive communication

Qualifications & Skills

• Nursing associate's or bachelor's degree
• RN licensure (single-state or multi-state)
• BLS certification
• Minimum of 1 year acute care nursing experience
• Excellent communication, assessment, organizational, and problem-solving skills
• Ability to work collaboratively, handle stress, and navigate regulations effectively
