Care Coordinator | RN/LPN (LOTUS)

About the role

As a Lotus Nurse | Care Coordinator (RN/LPN), you are responsible for the training and education of the team members that provide care and services to the Lotus Guests (rapid recovery and rehabilitation), as well as the care management of Lotus Guests.

Responsibilities

• Train and educate team members providing care and services to Lotus Guests
• Care management of Lotus Guests

Requirements

• Active, and in good standing, Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
• Immediate to advanced computer skills, including experience working with an EMR or EHR
• Extensive customer service experience required
• Prior experience in social services highly preferred
• Prior experience as an Intake Coordinator highly preferred
• Ability to make independent decisions required
• Maintain care and use of supplies and equipment
• Function independently and with personal integrity required
• Excellent written and oral communication skills
