Care Coordinator RN
About the role
The RN Care Coordinator will play a central role in patient care, assessing, planning, and facilitating comprehensive care across the continuum. You will advocate for patients while collaborating with various healthcare professionals to ensure timely and high-quality transitions.
Responsibilities
- Completes and documents a discharge planning assessment on identified patients, reassessing as necessary and updating the plan.
- Facilitates the development of a multidisciplinary discharge plan, engaging relevant health team members, the patient, and post-acute care providers.
- Oversees and evaluates the implementation of the discharge plan.
- Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources.
- Collaborates with healthcare team and post-acute service providers to ensure timely and smooth transitions to appropriate post-acute services.
- Identifies risk for readmission and implements interventions to mitigate these risks for at least a 30-day period.
Requirements
- Graduate of an accredited school of nursing and at least two (2) years of acute hospital clinical experience, or
- Masters in a case management or nursing field and one (1) year of experience, or
- Registered Nurse (RN) license in WA, or
- Basic Life Support (BLS) certification within 90 days, or
- Preferred: Bachelors of Science in Nursing, at least five (5) years of nursing experience, Certified Case Manager, and Accredited Case Manager certifications.

Pay
The posted compensation range is $50.25 - $97.78 per hour, reflecting the lowest to highest pay CommonSpirit Health believes it might pay for this position, based on the circumstances at the time of posting.
Schedule
This role is PRN (Per Request Not Scheduled), with varying hours and shifts.
