Care Coordinator RN
About the role
The RN Care Coordinator will play a pivotal role in patient care, overseeing comprehensive care plans and facilitating seamless transitions to post-acute services. This role requires strong clinical acumen, excellent communication and advocacy skills, and a strategic mindset.
Responsibilities
- Completes and documents a discharge planning assessment on identified patients, reassessing as needed and updating the plan.
- Facilitates the development of a multidisciplinary discharge plan, involving other health team members, the patient, and post-acute care providers.
- Oversees the implementation of the discharge plan and collaborates with the multidisciplinary team to ensure appropriate care progression and resource utilization.
- Collaborates with healthcare teams and post-acute service providers to ensure timely and smooth transitions to the most appropriate type and setting of post-acute services based on patient needs.
- Identifies and mitigates risks for readmission for at least a 30-day period.
Requirements
- Graduate of an accredited school of nursing.
- Minimum two (2) years of acute hospital clinical experience, or
- Masters in Case Management or Nursing field, and one (1) year of experience, or
- Registered Nurse (RN): TX, or
- Basic Life Support - CPR, within 90 Days.
Preferred
- Bachelor of Science in Nursing (BSN).
- At least five (5) years of nursing experience, or
- Certified Case Manager, or
- Accredited Case Manager.

Pay
The posted compensation range is $37.22 - $55.37 /hour, reflecting the reasonable estimate of what CommonSpirit Health might pay for this position, based on the circumstances at the time of posting.
Schedule
This role is full-time, with a weekly schedule of Monday through Friday from 8:00 AM to 4:30 PM.
