Care Coordinator RN
About the role
The RN Care Coordinator plays a central role in patient care, assessing, planning, and facilitating comprehensive care across the continuum. You will advocate for patients while collaborating with various healthcare professionals to ensure timely and high-quality transitions.
Responsibilities
- Completes and documents a discharge planning assessment on identified patients or upon request.
- Facilitates the development of a multidisciplinary discharge plan, engaging relevant health team members, the patient, and post-acute care providers.
- Oversees and evaluates the implementation of the discharge plan.
- Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources.
- Collaborates with healthcare team and post-acute service providers to ensure timely and smooth transitions to appropriate post-acute services.
- Identifies risk for readmission and implements interventions to mitigate these risks for at least a 30-day period.
Requirements
- Graduate of an accredited school of nursing.
- Minimum two (2) years of acute hospital clinical experience, or
- Masters in Case Management or Nursing field, and one (1) year of experience, or
- Registered Nurse (RN): TN, or
- Basic Life Support - CPR, within 90 Days.
Preferred
- Bachelor of Science in Nursing (BSN).
- At least five (5) years of nursing experience, or
- Certified Case Manager, or
- Accredited Case Manager.

Pay
The posted compensation range is $33.84 - $50.34 /hour, reflecting the lowest and highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
Schedule
This role is full-time with a weekly schedule of Monday - Friday (8:00am - 4:30pm) and operates on a day shift.
