Care Coordinator - Part-Time

Responsibilities

  • Identify seniors' health needs
  • Build caseload by interacting with residents
  • Communicate needs to Clinic Director or ADO
  • Support intake process for new residents
  • Schedule evaluations and assessments
  • Auxiliary client-facing services
  • Conduct wellness classes and personal fitness training
  • Auxiliary scheduling and team member support
  • Assist with reporting and billing
  • Perform general administrative support

Qualifications

  • Certification in healthcare, business administration, or related field
  • Two years of relevant experience in a senior living setting preferred
  • Proven experience in administrative and operational roles
  • Experience in a customer-facing role, focusing on exceptional service
  • Understanding of wellness offerings, physical, occupational, and speech therapy
  • Knowledge of laws and restrictions related to Assisted Living, Independent Living, and Memory Care
  • Exceptional communication and interpersonal skills
  • Passion for working with older adults and providing care
  • Ability to work well with others and follow direction
  • Proficiency in computer skills, including Internet browsers and Microsoft Office programs
  • Physical ability to lift/move 20lbs, maintain a stationary position, and use hands and fingers
