Care Coordinator - Part-Time

Responsibilities

  • Identify seniors' health needs
  • Build caseload by interacting with residents
  • Communicate needs to Clinic Director and/or ADO
  • Conduct intakes and perform wellness trainings
  • Support Clinic Directors in intake process
  • Schedule evaluations and assessments
  • Obtain orders and documents from providers
  • Provide care coordination services
  • Perform health screenings
  • Conduct wellness classes and personal fitness training
  • Auxiliary scheduling and team member support
  • Assist with onboarding new team members
  • Support interim Clinic Director duties
  • Send weekly census spreadsheets
  • Aid in weekly billing closeouts
  • Coordinate supply orders as requested
  • Other tasks as assigned

Qualifications

  • Certification in healthcare, business administration, or related field
  • Two years of relevant experience in a senior living setting preferred
  • Proven experience in administrative and operational roles
  • Experience in a customer-facing role, focusing on exceptional service
  • Understanding of wellness offerings, physical, occupational, and speech therapy for seniors
  • Knowledge of laws and restrictions regarding Assisted Living, Independent Living, and Memory Care
  • Exceptional communication and interpersonal skills
  • Passion for working with older adults and providing care
  • Ability to work well with others and take direction from management
  • Professional curiosity, desire to learn, and problem-solving skills
  • Ability to remain calm in stressful situations
  • Flexibility to meet patient/community needs
  • Proficiency in Internet browsers and Microsoft Office programs

  • Must have the ability to travel and commute between multiple assigned locations
