Responsibilities
- Identify seniors’ health needs by interacting directly with residents to proactively assess therapy needs and promote care coordination services.
- Communicate identified needs to the Clinic Director and/or Area Director of Operations to initiate screenings.
- Maintain an on-site presence in communities to build relationships with team members, residents, and community partners.
- Support Clinic Directors in completing the intake process for new residents, including gathering documentation, information, and obtaining consent from residents or Power of Attorney (POA).
- Assist in scheduling evaluations and assessments by clinical staff.
- Obtain orders and other required documents from providers.
- Provide care coordination services through touchpoint visits.
- Perform health screenings under the supervision of licensed therapists, adhering to federal, state, and local statutes.
- Conduct wellness classes, educational events, and personal fitness training.
- Assist Clinic Directors in coordinating therapists’ weekly schedules to optimize efficiency.
- Support the onboarding of new team members and serve as an interim Clinic Director to cover for PTO or open positions.
- Review operational reports and act on closing gaps in care coverage.
- Send weekly census spreadsheets to the Executive Director, Director of Wellness, Clinic Director, and Area Director of Operations.
- Assist with weekly billing closeouts.
- Coordinate with the Area Director of Operations to order supplies as requested and assist in completing other administrative tasks.
- Comply with HIPAA and PHI guidelines, ensuring confidentiality is always protected.
- Communicate professionally and maintain strong working relationships with physicians, care staff, and interdisciplinary teams.
- Demonstrate flexibility to meet patient and community needs.
- Perform other duties as assigned, consistent with qualifications, professional practices, and ethical standards.
Requirements
- Certification in healthcare, business administration, or a related field.
- Two years of relevant experience in a senior living setting preferred.
- Proven experience in administrative and operational roles, including managing schedules, handling correspondence, and maintaining records.
- Experience in a customer-facing role, with a focus on providing exceptional service to residents and their families.
- Solid understanding of wellness offerings, physical, occupational, and speech therapy for seniors, as well as general knowledge of laws and restrictions regarding Assisted Living, Independent Living, and Memory Care (preferred).
- Exceptional communication and interpersonal skills, with a passion for working with older adults and providing the best care possible.
- Ability to work well with others and take direction from management.
- Self-motivated, proactive, and results-driven with a strong sense of professional curiosity and a desire to learn and recommend solutions.
- Ability to remain calm in stressful situations, handle interruptions, and multitask effectively.
- Respect for patient/resident rights and confidentiality.
- Proficiency in internet browsers (e.g., Chrome, Edge) and advanced knowledge of Microsoft Office programs, including Outlook, Excel, and Word.

Pay
$17.00 - $22.00 per hour
