Care Coordinator (OhioRISE)

Location: Muskingum/Coshocton, OH. This role is eligible for a $3,000 sign on bonus.

Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual. OhioRISE is a Medicaid managed care program that provides specialized services for youth ages 0-20 with complex behavioral health and/or substance use needs. The OhioRISE Care Coordinator is responsible for creating, planning, and organizing a team as defined by the family. This position facilitates child & family team meetings that support youth at home, in school, and in their communities with the goal of keeping families together and reducing hospitalizations.

Responsibilities

• Uses a strengths based and solution focused mindset in all job functions and interactions internally and externally
• Develop, lead, and facilitate team meetings for the young person and their identified team.
• Provides care coordination within the family’s home, community, or preferred location
• Partners along with the young person, family, natural supports, and other service providers to identify care coordination needs/services to ensure the best outcomes for young people and families.
• Maintains required documentation to access and maintain OhioRISE benefits
• Meets quality and fidelity requirements as defined by ISBH OhioRISE standards
• Continually acquires and maintains knowledge of community resources.
• Attends mandatory in-person and virtual trainings and supervisions which will require travel
• Completes all assigned trainings within identified timelines and maintains required certifications
• Follows ISBH policies and procedures in maintaining confidentiality, professionalism, and incident reporting
• Utilizes databases and Electronic Health Record systems for accurate client documentation
