Care Coordinator (OhioRISE)

About the role

Integrated Services for Behavioral Health (ISBH) is a community‑minded, forward‑thinking behavioral health organization serving Southeastern and Central Ohio. The Care Coordinator works with children, youth, and families with significant behavioral health needs, providing service linkage, care coordination, and collaborative partnerships with community agencies.

Responsibilities

• Joins with family to identify care coordination needs/services in line with service delivery standards and program outcomes.
• Works with families to define cultural factors that influence strengths, functioning, and family interaction styles.
• Identifies strengths of children, youth, and families for utilization in care coordination.
• Coordinates family‑based services for children, youth, and families in home, school, and community settings.
• Ensures services identified on care plans are the most appropriate, least restrictive, and meet safety and treatment needs.
• Engages and builds positive relationships with children, youth, and families in coordination with child and family teams.
• Develops collaborative and creative partnerships with community resources to meet diverse needs.
• Maintains necessary documentation, participates in program evaluation, attends team and program planning meetings, cross‑systems training, and acquires knowledge of community resources.
• Remains current with all training requirements, including High Fidelity Wraparound, MI, Cultural Humility, etc.
• Performs other duties as assigned.

Requirements

• Experience providing services/support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services/behavioral healthcare: three years with a high school diploma or equivalent; or two years with an associate or bachelor’s degree; or one year with a master’s degree or higher.
• Knowledge and experience in High‑Fidelity Wraparound preferred (certification provided at time of employment).
• Two years of experience in a coordinated supportive services or care coordination role preferred.
• Experience working with people with autism spectrum disorders and developmental disabilities preferred.
• Experience in one or more of: family systems, community systems and resources, case management, child and family counseling/therapy, child protection, child development.
• Culturally humble or responsive with training and experience to manage complex cases.
• Qualifications and experience needed to work with children and families experiencing serious emotional disturbance (SED), trauma, co‑occurring behavioral health disorders, and involvement with one or more child‑serving systems (e.g., child welfare, I/DD, juvenile justice, education).
• Excellent organizational skills with ability to stay focused and prioritize multiple tasks.
• Demonstrates a high degree of cultural awareness.
• Experience with multi‑need individuals and families; broad knowledge of community service systems.
• Willing to participate in and lead cross‑systems care coordination.
• Able to communicate effectively verbally and in writing.
• Must be able to operate in an Internet‑based, automated office environment.
• Valid Driver’s License required.
• Ability to operate, receive, and conduct video calls/meetings in an automated office environment required.
• Ability to maintain a minimum of 100 Mbps high‑speed internet connection required.

Pay

The pay range for this position is $20.19 – $25.03 per hour, based on experience, education, and/or licensure. This role is eligible for a $3,000 hiring bonus.
