Care Coordinator (LMSW) - Social Work

Location: 435 Lewis Avenue, Midstate Medical Center (10123). Shift includes rotating weekends and holidays.

About the Role

The social worker is a graduate-level professional with demonstrated expertise in a healthcare setting, specializing in the assessment and treatment of patients across the continuum of care. This role involves developing and implementing discharge planning within an interdisciplinary healthcare team, addressing complex clinical needs by identifying and removing barriers to post-acute care access. The social worker collaborates with providers to optimize efficient use of acute care hospital days, prevent prolonged length of stay, and address all aspects of a patient’s psychosocial needs. Responsibilities include leading family meetings, providing bereavement support and grief counseling, advocacy with community agencies, and addressing substance abuse. The role requires adherence to ethical principles and professional standards of practice in line with the NASW code of ethics.

Responsibilities

• Develop and implement discharge planning within an interdisciplinary healthcare team.
• Identify and remove barriers to post-acute care access.
• Collaborate with providers to optimize efficient use of acute care hospital days and prevent prolonged length of stay.
• Address complex psychosocial needs, including leading family meetings and providing bereavement support and grief counseling.
• Advocate with community agencies and within the acute care interdisciplinary team.
• Address substance abuse issues and other barriers to patient care.
• Work collaboratively with all hospital teams to optimize patient quality of life by addressing needs holistically.
• Maintain ethical principles and professional standards of practice in adherence to the NASW code of ethics.
• Provide expert verbal and written clinical documentation and consultation along the continuum of care.

Requirements

• Master’s in Social Work (MSW).
• Valid Master of Social Work License with the State of Connecticut.
• Knowledge of Connecticut Social Work Law for mandated reporting of child abuse and neglect, as well as mandated reporting for elderly and individuals with intellectual disabilities.
• Comprehension of medical terminology, natural history of illness, and general disease processes.
• Ability to rely on educational resources to continuously improve clinical practice.
• Excellent communication, negotiation, and conflict resolution skills.
• Ability to work collaboratively, efficiently, and effectively with multidisciplinary healthcare professionals.
• Ability to multi-task and address the needs of healthcare team members, patients, and families.
• Experience addressing complex psychosocial needs by working with community resources and removing barriers to optimize health and quality of life.
• Ability to work in a fast-changing healthcare environment.
• Adherence to the NASW Code of Ethics.

Qualifications

• Preferred previous experience in a healthcare setting.
• Knowledge of computer applications preferred.
