Licensed Master Social Worker (LMSW) - Client Care Coordinator

About the Role
Our healthcare staffing team is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join a Family with Children Shelter in the Bronx. This is an excellent contract-to-hire opportunity for a dedicated clinician who is passionate about supporting families experiencing homelessness and helping them access critical mental health and community resources. In this role, you will serve as a Client Care Coordinator, providing clinical assessments, crisis intervention, care coordination, and ongoing support while collaborating with shelter staff and community partners to improve outcomes for families.
Responsibilities
- Complete biopsychosocial assessments and mental health/substance use screenings.
- Conduct crisis intervention and provide short-term supportive counseling.
- Develop individualized service plans and maintain accurate case documentation.
- Facilitate case conferences and collaborate with interdisciplinary teams.
- Provide psychoeducation and meet with families to review progress and goals.
- Coordinate referrals to community-based mental health, medical, and social service providers.
- Collaborate with child welfare agencies (including ACS when applicable).
- Provide clinical guidance and training to shelter and social service staff.
- Lead support groups, workshops, and educational sessions.
- Participate in incident reviews, quality assurance activities, and chart audits.
- Build and maintain strong partnerships with community organizations.
- Ensure compliance with HIPAA, agency policies, and documentation standards.
Requirements
- Master's Degree in Social Work (MSW)
- Active New York State Licensed Master Social Worker (LMSW)
- Minimum of 2 years of social services experience
- Experience working with one or more of the following populations:
- Families experiencing homelessness
- Mental health
- Substance use disorders
- Domestic violence
- Child welfare
- Public benefits and entitlements
Preferred Qualifications
- Experience working in a homeless shelter, supportive housing, or community-based social services
- Bilingual (Spanish) strongly preferred
- Knowledge of the CARES system is a plus
Skills
- Strong clinical assessment and crisis intervention skills
- Excellent communication and documentation abilities
- Ability to manage multiple priorities in a fast-paced environment
- Collaborative, compassionate, and solution-oriented approach
- Proficiency with Microsoft Office (Word and Excel)
- Knowledge of HIPAA and trauma-informed care principles

Schedule
Monday-Friday with flexible start times (9:00 AM - 5:00 PM or 10:00 AM - 6:00 PM)
Pay
$40.00 - $43.00 per hour
