Care Coordinator (LCSW) - Social Work

Location: 435 Lewis Avenue, Midstate Medical Center (10123). Shift: Weekend and weekday availability for as-needed hours.

About the role

The social worker is a graduate-level professional with demonstrated expertise in the assessment and treatment of patients along the continuum of care. As a key member of the interdisciplinary healthcare team, you will identify risk factors for psychosocial interventions and discharge needs, address discharge barriers, and develop and implement safe discharge plans—regardless of complexity—in a timely manner. Responsibilities include addressing all aspects of patients' psychosocial needs, such as leading family meetings, providing bereavement support and grief counseling, advocacy with community agencies, and addressing substance abuse. You will collaborate with all hospital teams to optimize patients' quality of life by addressing their needs holistically while maintaining ethical principles and professional standards of practice in adherence to the NASW code of ethics.

Responsibilities

• Assess and treat patients along the continuum of care, identifying psychosocial risk factors and discharge needs.
• Develop and implement safe, timely discharge plans, addressing barriers regardless of complexity.
• Lead family meetings and provide bereavement support, grief counseling, and advocacy with community agencies.
• Address substance abuse and other complex psychosocial needs by collaborating with community resources.
• Work collaboratively with multidisciplinary healthcare teams to ensure seamless transitions of care.
• Maintain ethical principles and professional standards in accordance with the NASW Code of Ethics.
• Provide expert verbal and written clinical documentation and consultation.
• Multi-task effectively to address the needs of healthcare team members, patients, and families.
• Navigate a fast-changing healthcare environment while working independently across the acute care setting.

Requirements

• Master’s in Social Work (MSW).
• Valid Licensed Clinical Social Worker (LCSW) credential with the State of Connecticut.
• Knowledge of Connecticut Social Work Law for mandated reporting of child abuse, neglect, elderly abuse, and mental health concerns.
• Understanding of medical terminology, disease processes, and reliance on educational resources to improve clinical practice.
• Excellent communication, negotiation, and conflict resolution skills.
• Ability to work collaboratively, efficiently, and effectively with multidisciplinary healthcare professionals.

Skills

• Strong clinical documentation and consultation skills.
• Ability to multi-task and address complex psychosocial needs.
• Proficiency in computer applications preferred.
• Bilingual in Spanish/English preferred.
