Care Coordinator - Flagstaff, AZ; Families FIRST; DCS

About the role

The Families FIRST program at Terros Health assists families involved with the Department of Child Safety. The Care Coordinator works closely with DCS, the clinical team, and other providers to coordinate and help families be successful in their DCS case plans.

Responsibilities

• Conducts activities as indicated on the “New Client Checklist”
• Completes a Short-Term Plan with assessed patients
• Conducts a meeting or conference call with the Service Team to discuss and finalize the Service Plan
• Schedules and conducts a monthly Service Plan review with the Service Team
• Makes sure the drug testing plan is monitored and updated
• Provides concrete supportive services to help families achieve sobriety
• Coordinates outreach and re-engagement activities with a Recovery Coach
• Completes required documentation in patient records
• Visits and provides services to patients in various locations

Requirements

• Professional, friendly, self-starter, organized, and compassionate

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field
• Experience working with families and substance abuse issues
• Ability to drive to various locations

Skills

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of substance abuse treatment and recovery programs
