Reports to the Executive Director/Administrator.
Responsibilities
- Ensure Resident Care Services comply with Victorian Senior Care Policy, DMA Clinical Coverage Policy 3L, and NC DHSR Licensure requirements.
- Apply working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirements for operating an Adult Care Home.
- Oversee Personal Care Service and Personal Care Service Documentation.
- Train staff on personal care services, resident needs, and ensure staff provide required care based on current service plans/care plans and aide task worksheets.
- Oversee Medication Administration and Medication Administration Records.
- Provide Medication Administration training per NC Adult Care Home Rules and Regulations and VSC Policy and Procedures.
- Coordinate and follow up on Licensed Health Professional Support Tasks, Pharmacy Reviews, Physician Referrals, or other outside referral agencies.
- Ensure FL2, Care Plans, physician orders, and other applicable documents are current and up to date based on residents’ needs.
- Monitor skin care, nail care, vitals, intake, and output of residents.
- Supervise direct care staff and medication aides through coaching, mentoring, and hands-on training.
- Provide hands-on resident care as needed or warranted.
- Responsible for rotating on-call (may include nights, weekends, and holidays).
- Continuously evaluate the Resident Care Program using quality enhancement tools and customer feedback to meet evolving resident needs.
- Support and actively participate in census-building initiatives, event hosting, and program presentations as requested by management.
- Maintain positive relationships with residents’ families/responsible parties and provide guidance on assisted living care and placement processes.
- Link families/responsible parties to support groups and outside resources when needed.
- Perform other LPN/RN duties allowed per NC Board of Nursing.
- Be familiar with Residents' Bill of Rights and assist residents in exercising those rights.
- Notify the Executive Director of any concerns of neglect/abuse/exploitation by other staff.
- Follow all regulations required by Federal HIPAA Laws.
- Perform other duties as required by circumstances.
Requirements
- NC Licensed Practical Nurse or NC Licensed Registered Nurse in good standing with the NC Board of Nursing.
- Current, unencumbered license in North Carolina with two years of clinical or direct patient care experience in a healthcare, home care, or long-term care setting.
- Valid NC Driver's License.
- Must obtain Medication Aide certification and medication aide training within 30 days of hire.
- Able to lift, push, and pull a minimum of 30 pounds.
- Able to bend, stoop, squat, and reach overhead as needed.
- Able to read, write, understand, and follow directions.
- No substantiated findings on the NC Personnel Registry or Health Care Program.
- Clear SBI and DMV record, maintained during employment.
- Two-step tuberculin skin test (first test prior to hire and second test within two weeks of the first test).
- Able to relate positively to residents, families, and work cooperatively with employees and vendors.
- CPR Certified.
Preferred Qualifications
- Working knowledge of Personal Care Services and the 3L Policy or experience in assisted living.
- Working knowledge of Adult Care Home Policies and Procedures and NC DHSR requirements.
- One year of experience working with residents who have dementia or a dementia-related diagnosis.
- Two years of supervisory/management experience in an Adult Care Home or similar setting.
Pay based on experience.
