Job Summary
The Care Coordinator serves as a primary point of contact for patients throughout their care journey. This position coordinates scheduling, communications, pre-procedure preparation, medical record collection, and other administrative activities that support the surgery center and provider care teams. The Care Coordinator works collaboratively with patients, providers, outside facilities, and other departments to promote an exceptional patient experience, facilitate efficient care coordination, and support positive clinical outcomes.
Essential Functions
- Schedules surgeries, procedures, and related appointments; serves as the primary contact for procedure and injection scheduling.
- Collaborates with providers, physicians, and care teams regarding treatment plans, scheduling needs, patient education materials, and coordination of care.
- Affords assistance to patients with completing required forms and documentation.
- Conducts outreach to patients, providers, pharmacies, and outside facilities as needed to support patient care coordination.
- Cross-trains in other departments and provides operational support as needed.
- Coordinates patient care activities with internal departments and external healthcare facilities.
- Conducts pre-procedure screenings and provides patient education regarding upcoming procedures.
- Maintains patient tracking systems and coordinates follow-up activities as needed.
- Obtains, reviews, and coordinates the transmission of medical records and other documentation necessary for patient care.
- Manages anticoagulant hold requests, facilitating timely review and completion through coordination with patients, prescribing providers, and pharmacies.
- Prepares and distributes pre-operative information packets and instructions to patients.
- Crosses-trains in other departments and provides operational support as needed.
- Complies with organizational policies and HIPAA regulations to maintain the confidentiality and security of patient information.
- Performs miscellaneous job-related duties and tasks as assigned.
Qualifications
- High school diploma/GED plus one year of medical clinic experience. OR Associate’s or Bachelor’s degree (preferred).
- Minimum of one year of strong customer service with an exceptional ability to listen and communicate with others required.

Work Environment
While performing the duties of this job, the employee regularly works in an ambulatory surgery center/clinic setting. This position is 100% in-office due to the nature of the responsibilities.
Physical Demands
- While performing the duties of this job, the employee is occasionally required to walk; sit; use hands to grasp or handle objects, tools, or controls; reach with hands and arms; stoop, kneel, or crouch; talk or hear.
- The employee must occasionally lift or move up to 20 pounds.
- Employee will be walking or standing for 40% of the time in this role and will be sitting for 60% of the time in this role.
- Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
Compensation & Work Schedule
- Full-time: 40 hours/week.
- Non-Exempt.
- Day Shift: Monday - Friday, 8:00AM - 4:30PM.
- No evenings or weekends.
