CARE COORDINATOR

About the role

Stonington Institute, a division of Universal Health Services, Inc. (UHS), has an immediate opening for a Care Coordinator. Stonington Institute provides substance abuse treatment options for adults in Southeastern Connecticut and surrounding areas, including medical detox, residential addictions treatment for active duty and retired military service members, partial hospitalization, and intensive outpatient treatment.

Responsibilities

• Completes a Discharge Plan for each client admitted to the Detox Program, documenting progress daily on approved forms and assuring completion of Discharge Checklists, “Risk of Overdose” Form, Basis 32, and Satisfaction Survey for each client.
• Affirms appropriate ROIs are completed for referral agencies and others as required.
• Maintains the Step-Down process to provide continuity of care.
• Collaborates with clients and members of the treatment team to incorporate client needs and preferences and the recommendations of the multidisciplinary treatment team into the discharge planning process.
• Acts as a liaison with community providers and responds to community inquiries related to discharge planning processes and procedures.

Requirements

• Bachelor’s degree required.
• Three (3) years’ experience in the substance abuse/mental health treatment field preferred.
• Previous case management/discharge planner experience required.
