About the Role
Sierra Health and Wellness uses evidence-based methods to restore families, relationships, and self-worth, transforming lives affected by substance abuse and mental health issues. We are seeking a Care Coordinator - SUD Counselor to join our team in assessing and monitoring clients' behaviors during treatment for chemical and alcohol abuse and dependency.
Responsibilities
- Assess clients' immediate and long-term needs and develop treatment plans to reduce or eliminate substance abuse and criminal thinking.
- Make referral appointments for mental health or medical needs, and collaborate with the Job Developer to enhance clients' employability or initiate SSI processes as needed.
- Document client behavior in group participation and counseling sessions.
- Provide crisis management, counseling services, and group therapies in a therapeutic setting, including education for clients and their families/significant others.
- Care for clients' emotional needs and coordinate treatment and discharge planning.
- Review client charts for completion of paperwork and signatures, and conduct weekly audits.
- Facilitate discharge planning and community outreach.
- Respond to emergencies and client complaints efficiently.
- Meet with clients individually at least twice a week and complete notes for every group and interaction.
- Attend staff meetings and trainings.
Qualifications
- High School diploma or GED.
- Certification as a Drug and Alcohol Counselor by a California Regulatory Agency.
- Minimum one year of experience in Drug and Alcohol counseling.
- Familiarity with HIPAA, client confidentiality, and personal rights.
- Valid License and/or certification in drug and alcohol counseling through an approved Department of Health Care licensing board.
- Current CPR and First Aid certification.
- Valid Driver’s License and insurable under Sierra Health and Wellness Centers’ automobile insurance policy.
Preferred Qualifications
- Two to five years of experience in drug and alcohol counseling.
- Bachelor’s degree in a related field.

Hourly: $25.00 - $35.00
