About the role

Sentara Health Plans is hiring a full-time (40 hours/week) Care Coordinator to perform both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+), and Substance Abuse Disorders. Standard working hours are 8am to 5pm EST, Monday through Friday.

Applicants must be located in Central Virginia (Farmville, Kenbridge). Travel may include up to 90 minutes one way.

Responsibilities

  • Perform a variety of casework duties and provide case management services to patients, families, and designated caregivers.
  • Develop, participate in, and monitor multidisciplinary collaboration of services to patients where appropriate.
  • Assist adult patients and their families with personal and environmental difficulties associated with medical conditions, up to and including terminal illnesses.

Requirements

Must possess a degree in Health & Human Services or one of the following related fields:

  • Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)
  • Counselor Education
  • Early Childhood Development
  • Educational Psychology

All degrees must be from schools accredited by the U.S. Department of Education. Degrees obtained outside the U.S. will be reviewed individually.

Experience

  • Long Term Care: 1 year (required)
  • Health Plan: 1 year (preferred)
  • Medicaid: 1 year (preferred)
  • Mental health experience: 1 year (preferred)
  • Experience with Children/Adolescents: 1 year (required)
