Care Coordinator
About the role
Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. We’re currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitate communication between providers, and ensure individuals receive the appropriate care for their individual needs.
Responsibilities
- A Bachelor’s degree in social work, human services, or a related field is required
- Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required
- An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required
Requirements
The ideal candidate will be able to work independently and as part of a team. They should have excellent communication skills and the ability to manage multiple tasks effectively.

Pay
$22.00-$26.00 per hour
Schedule
Monday-Friday 8:00am-5:00pm
