Care Coordinator
About the role
Seasons is a comprehensive behavioral health center providing services to communities in Northwest Iowa. We are seeking a full-time Care Coordinator to help coordinate service provision for providers, facilitate communication, and ensure individuals receive appropriate care.
Responsibilities
- Coordinate service provision for providers
- Facilitate communication between providers and individuals
- Ensure individuals receive appropriate care based on their needs
Requirements
- A Bachelor’s degree in social work, human services, or a related field is required
- Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required
- An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required

Pay
$22.00-$26.00 per hour
Schedule
Monday-Friday 8:00am-5:00pm
Qualifications
Required: Bachelor’s degree in social work, human services, or a related field. Preferred: Active Iowa BSW license.
Skills
Strong communication skills, ability to manage multiple tasks, and experience working with individuals with behavioral health concerns.
