Responsibilities
- Facilitates the quality and improvement of a person’s experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.
- Provides coordination of care of services and resources available internally as well as throughout the community.
- Acts as an advocate for the needs and rights of individuals.
- Affords assistance in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community.
- Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members.
- Keeps abreast of all federal and state regulations and laws and all Recovery Innovations policies and procedures as they presently exist and as they change or are modified.
- Understands and adheres to Recovery Innovations compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.
- Cross Functional Collaboration: Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner. Partners with other provider agencies in the coordination of care to help participants achieve wellness.
- Performs other related duties as assigned.
Requirements
- Education/Licenses: Bachelor’s Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or Bachelor’s Degree in any field and a minimum of two (2) years of behavioral health experience; or High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience.
- Experience: Ability to provide courteous assistance in a concise, informative manner, keeping a positive tone and appropriate non-verbal language. Works cooperatively in a team environment.
- Skills: Excellent computer skills, including Microsoft Office Suite. Demonstrated ability to effectively communicate both verbally and in writing. Excellent interpersonal skills. Excellent customer service skills. Demonstrated organizational skills and high degree of accuracy. Must be able to pass de-escalation training, as defined by state requirements. Driving Requirements: If driving is required for this position, must meet all requirements outlined in Recovery Innovations driving policy. If not qualified to drive per driving policy, a Non-Driver Attestation form must be signed.
- Miscellaneous Requirement(s): Travel required based on business need. Applicants must be at least 18 years old unless residing in a state where funder or government regulations set a different minimum age requirement. Must be able to pass a seven-year background check. Must be able to pass substance abuse testing and/or tuberculous testing (if applicable). Must be able to pass education verification check. Must be able to obtain designated fingerprinting clearance in applicable states. Must be able to pass three-year motor vehicle report and meet minimum eligibility requirements criteria of insurance carrier (if driving is required for this position).
