Care Coordinator
Position Summary
Power Health Colorado (PHCO) is seeking a highly organized, proactive, and patient-focused Care Coordinator to support the continuity of care for both Personal Injury (PI) and Workers’ Compensation (WC) patients. This role serves as a critical communication and operational bridge between patients, providers, referral partners, attorneys, payers, employers, and internal clinical teams.
Key Responsibilities
Review and monitor PI and WC patient cases to ensure treatment compliance and continuity of care
Identify gaps in treatment, missed appointments, delayed referrals, or barriers preventing patient progression
Communicate with patients regarding care plans, scheduling needs, treatment expectations, and follow-up requirements
Maintain ongoing communication with attorneys, case managers, employers, adjusters, nurse case managers, and referral partners regarding patient status and care progression
Escalate concerns, delays, or compliance issues to leadership and providers appropriately
Ensure patient charts, authorizations, liens, and case documentation remain accurate and updated
Aid in coordinating multidisciplinary care between medical providers, physical therapy, chiropractic, imaging, specialty referrals, and ancillary services
Track pending referrals, diagnostic orders, and treatment recommendations to ensure completion
Process outgoing referrals to external specialists, imaging centers, diagnostic facilities, and other healthcare providers
Obtain and verify authorizations, approvals, and supporting documentation required by payers or referral destinations
Communicate with insurance companies, WC adjusters, TPAs, attorneys, and referral offices regarding referral status
Ensure clinical documentation, orders, and supporting records are transmitted accurately and timely
Follow up on pending or delayed referrals to prevent interruptions in patient care
Maintain referral tracking systems and provide status updates to providers and leadership
Manage incoming referrals from external providers, attorneys, employers, and referral sources
Contact patients promptly to schedule appointments and coordinate care
Follow up consistently with patients who have not yet scheduled or who have pending appointments
Verify demographics, insurance information, claim details, and case information during intake
Absorb and understand appointment expectations, required documentation, and next steps
Work closely with front office and clinical teams to optimize scheduling and patient flow
Assist in reducing no-shows, cancellations, and scheduling delays through proactive communication
Qualifications
Minimum 2 years of experience in healthcare coordination, medical office operations, case management, referrals, or related healthcare setting preferred
Experience with Personal Injury and/or Workers’ Compensation strongly preferred
Strong organizational skills with the ability to multitask in a fast-paced environment
Excellent communication and customer service skills
Strong critical thinking and problem-solving abilities
Ability to maintain professionalism and urgency while managing sensitive situations
Familiarity with EMR/EHR systems preferred (eClinicalWorks experience a plus)
Knowledge of insurance verification, authorizations, referrals, and healthcare workflows preferred
Bilingual English/Spanish preferred but not required
